FDCSD 7th-12th grade athletic physicals March 4

Athletic physicals for current 7th-12th-grade students will be offered from 5:15-6 p.m. Thursday at Fort Dodge Senior High.

The cost is $15. Masks will be required for all students at all times unless specifically asked by a healthcare provider to remove it during the physical.

All 7th-12th grade Dodger student athletes are encouraged to attend.

A sports physical needs to be current and on file in the nurse’s office prior to the first day of practice of the athlete’s sport season. Physicals must stay current through the end of the athlete’s sport season. This includes for seniors who participate in summer sports. Physicals are good for one year from the date they are completed.

FDSH students can have their pre-workup (height, weight, heart rate and blood pressure) done in the school nurse’s office during lunch on March 2 and 3. FDMS seventh- and eighth-grade students and any at-home learners will have their pre-workups completed the night of physicals.

The physical form is available at fdschools.org under Athletics near the bottom of the page. It is also available in the nurse’s office and the main office at both the middle and high schools. The front of the form must be completed and signed by a parent/guardian, and brought with the athlete on March 4 in order for the physical to be completed. Payment by cash or check is also required at the time of the physical.

Current sixth-grade students interested in participating in athletics next year will need to have a physical completed by their primary care provider prior to starting practice for their sport.
