LAKE CITY — UnityPoint At Home Homecare and Hospice has transitioned to Storm Lake and Fort Dodge as the home office for leadership and administrative support.

That included the closing of the Lake City office space for home care and hospice. Any new hospice referrals residing in the updated service area are being admitted under the Fort Dodge hospice office or the Storm Lake homecare office.

UnityPoint Hospice will continue to provide the same quality care using Fort Dodge as the home office. The counties served are: Buena Vista, Sac, Pocahontas, Calhoun, Humboldt, Webster, Wright, Hamilton, and the northern areas of Boone and Greene counties.

Stewart Memorial Community Hospital and UnityPoint Hospice will continue to partner to provide respite and general inpatient levels of care for patients.
